Case Manager (Healthcare)

Location: Remote position supporting members in Viroqua, Wisconsin and the surrounding area; up to 40% travel for in-person member visits and occasional office training or meetings. Monday-Friday, 8:00 am-4:30 pm CST.

Salary: $53,700-$72,600 per year.

What you will do

• Assess members' health, safety, strengths, interests, and preferences and develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses to deliver services in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at members' residences.
• Complete quarterly in-person visits and maintain monthly phone contact.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, ensure cost-effective services, and maintain case notes, authorizations, and care plan updates.

Requirements

• Bachelor's degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor's degree in another field plus at least three years of relevant experience.
• Valid state driver's license and personal vehicle liability insurance with minimum 100/300/100 limits required.
• Ability to conduct regular in-person visits and travel up to 40% within the assigned coverage area.
• Work must be performed from a dedicated space protecting member PHI and HIPAA information; home internet must provide at least 25 Mbps download and 10 Mbps upload speeds.
• Patient-facing employees must complete TB screening if selected.

Nice to have

• Residence within 45 minutes of the assigned coverage area.
