TL;DR
RN Clinical Documentation Specialist - Referrals (Healthcare Revenue Cycle): Reviewing patient medical records and improving clinical documentation to support accurate diagnoses, DRG assignments, reimbursement, and appeals with an accent on physician clarification, coding accuracy, and regulatory compliance. Focus on evaluating severity of illness and risk of mortality, assigning working ICD-10-CM/PCS codes, and resolving documentation gaps and payer denials.

Location: Putnam, Connecticut, United States; residence and work authorization in the United States are required.

Base salary: $80,000 per year

What you will do

  • Review patient medical records within 24–48 hours of admission and conduct subsequent reviews.
  • Evaluate severity of illness, risk of mortality, principal diagnoses, complications, comorbidities, and working DRG assignments.
  • Assign working ICD-10-CM and PCS codes using the CDE software encoder.
  • Clarify missing, unclear, unsupported, or conflicting documentation with physicians through written and face-to-face communication.
  • Educate physicians, nurses, advanced practice providers, care coordinators, and other healthcare professionals on documentation guidelines.
  • Partner with coding and clinical teams to prepare payer appeals, resolve root causes, and minimize denials.

Requirements

  • Current RN licensure.
  • At least five years of acute care nursing experience in medical/surgical, intensive care, or emergency department settings.
  • Strong verbal and written communication skills, physician relations, organization, prioritization, and computer skills.
  • Ability to maintain clinical documentation regulatory guidelines and present information clearly.
  • Must reside in and be authorized to work within the United States.
  • CRCR certification required within nine months of hire.
