RN Assessment Nurse (Clinical Assessments)

Location: MA - Barnstable; hybrid

Salary: $95,000–$104,000 per year

What you will do

• Complete MDS and comprehensive member assessments during intake within the assigned territory.
• Conduct self-efficacy and condition-specific screeners, including PHQ-9, GAD-7, AUDIT, DAST-10, and behavioral health assessments.
• Develop care plans with members, document clinical concerns, and escalate urgent needs to the appropriate departments.
• Triage members for behavioral health programming and conduct in-person clinical exams when appropriate.
• Collaborate with the care team to determine appropriate program placement and care intensity.
• Perform medication reconciliation, address adherence concerns, and close quality gaps through accurate chart documentation and coding.

Requirements

• Graduate of an accredited nursing school and licensed as a registered nurse.
• 3+ years of relevant experience.
• Strong critical thinking, independent clinical judgment, and ability to identify barriers and develop practical solutions.
• Strong written and verbal communication across phone, text, virtual, and in-person settings.
• Experience using technology to engage members remotely and document interactions in electronic health records and scheduling platforms.
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles.
