Nurse Navigator (RN) (Oncology): Coordinating patient care across the full cancer continuum in a community oncology clinic with an accent on patient education, care access, psychosocial support, and multidisciplinary collaboration. Focus on identifying gaps in care, leading patient care conferences, monitoring oral chemotherapy adherence and side effects, and supporting survivorship and advance care planning.
Location: Austin, Texas, supporting the Texas Breast Specialists Austin Midtown location. Full-time, Monday–Friday, 8:30 a.m.–5:00 p.m.; no weekends, call, or major holidays.

What you will do
- Coordinate patient care throughout the cancer care continuum and help patients, families, and caregivers overcome healthcare system barriers.
- Identify gaps in care delivery, develop action plans, and coordinate referrals and resources with multidisciplinary teams.
- Organize and lead regular patient care conferences and communicate outcomes to physicians, clinical staff, and practice leadership.
- Assess patient risk, provide basic psychosocial support, and arrange additional screening or support services.
- Provide or support oral chemotherapy education, adherence support, and side-effect monitoring.
- Participate in survivorship, advance care planning, research, quality improvement, navigation metrics, patient advocacy, and community outreach.
Requirements
- Bachelor of Science in Nursing and Registered Nurse licensure.
- At least five years of Registered Nurse experience in an oncology practice or clinic.
- Ability to apply the full nursing process, including assessment, nursing diagnosis, care planning, implementation, and outcomes evaluation.
- Strong leadership, collaboration, organization, critical thinking, and verbal and written communication skills.
- Ability to coordinate care with multidisciplinary teams, assess health disparities, and identify community resources.
- Working computer, internet, and electronic medical record system skills.
Nice to have
- Oncology Certified Nurse certification.
- Nurse Navigator certification.
