What you will do
- Support transitions of care through telephonic patient outreach, post-discharge follow-up, and coordination across hospitals, provider offices, and community agencies.
- Arrange appointments, referrals, transportation, prescriptions, homecare services, and other support specified in patient care plans.
- Create and distribute daily admission, discharge, emergency department, urgent care, and patient activity reports.
- Review referrals and hospital admissions, track level-of-care determinations, and identify authorization, denial, pending, and payment-day information.
- Monitor emergency department utilization and communicate information to clinical staff for timely intervention.
- Maintain patient registries, spreadsheets, referral databases, and program documentation while collaborating with resource teams and affiliated organizations.
Requirements
- High school diploma or GED.
- At least two years of experience in a clinical setting, such as a nursing unit, care management department, or physician’s office.
- Experience with electronic medical records and general computer systems.
- Strong working knowledge of Microsoft Office, including Word and Excel.
- Customer-focused interpersonal, organizational, prioritization, negotiation, and multitasking skills.
- Ability to work autonomously and effectively in a dynamic, high-stress healthcare environment with patients, families, providers, and interdisciplinary teams.
Nice to have
- College degree.
- Medical Assistant certification.
- Three years of related experience.
