Manager, TOC Social Work Case Management (Healthcare)

Location: Remote USA; Monday–Friday, 8:30am–5:00pm in CST, MST, or PST hours

Salary: $73,000–$110,000 annually

What you will do

• Build, develop, and lead a high-performing team of Social Worker Case Managers and behavioral health care coordinators.
• Track staff performance through competency check-offs, audits, shadowing, and 1:1s, providing coaching where needed.
• Create workflows, assessments, and plans of care that improve team efficiency and patient outcomes.
• Support escalated patient needs and lead onboarding and ongoing education.
• Partner with interdisciplinary and cross-functional teams to standardize workflows across programs.
• Spend 30–40% of the time on direct patient outreach and case work.

Requirements

• Active social work license: LMSW or higher.
• At least 3 years of case management experience.
• At least 3 years of people management experience, including developing and retaining high-performing teams.
• Experience working with interdisciplinary care management teams; management experience is preferred.
• Ability to independently work a fully remote schedule from the USA during CST, MST, or PST business hours.
