Home Health Registered Nurse, RN, Case Manager (Home Health)

Location: Lowell, Massachusetts; position based in the community and patients' homes. Full-time daytime schedule, 40 hours per week, with weekend and holiday commitment.

Pay range: $84,300.22–$107,481.20 per year.

What you will do

• Assess patients' physical and psychosocial needs and deliver comprehensive home health care.
• Develop treatment plans and realistic care goals with patients, families, physicians, and other healthcare providers.
• Manage an assigned caseload, establish daily work plans, and prioritize patient and family needs.
• Perform telephone triage, respond to urgent messages and visits, and provide patient and caregiver advocacy.
• Complete physicians' orders, OASIS documentation, medication reconciliation, and levels-of-care documentation.
• Coordinate care, discharge planning, case conferences, and crisis interventions with internal teams and community resources.

Requirements

• Massachusetts RN licensure required.
• Valid state-issued driver's license required.
• At least one year of acute medical/surgical nursing experience.
• Current CPR certification.
• Knowledge of home health and hospice standards, symptom management, pharmacology, and medication administration.
• English communication, reading, and writing skills required; work involves autonomous home-care practice and field safety.
