Location: Manchester, NH, with in-home care across Manchester and surrounding communities

provides 24/7 in-home healthcare support through the VNA of Manchester and Southern New Hampshire.
What you will do
- Perform comprehensive assessments of patients in their homes.
- Complete medication reconciliation and provide medication and disease-process education.
- Deliver wound and ostomy care.
- Provide home infusion therapy.
- Perform skilled nursing interventions, including pleural drainage and urinary catheter management.
- Coordinate individualized care with patients, caregivers, ambulatory providers, and an interdisciplinary care team.
Requirements
- Registered Nurse position providing in-home patient care in Manchester and surrounding communities.
- Availability for per diem scheduling.
- Commitment to working every other weekend, holidays, and on-call shifts.
- Ability to provide skilled nursing interventions and patient education in a home health setting.
