Clinical Nursing Manager- Acute Care Transition Team

Pay range: $134,525.66–$171,524.44 per year

What you will do

• Oversee daily clinical and administrative operations for the Acute Care Transition Team.
• Lead inpatient nursing care for patients awaiting an inpatient bed and coordinate safe transitions across the care journey.
• Prepare staff schedules, allocate resources, and coordinate services to optimize patient care and patient flow.
• Supervise, evaluate, coach, interview, orient, and support the professional development of nursing staff.
• Participate in interdisciplinary rounds, discharge planning, committees, councils, and multidisciplinary projects.
• Develop budgets, policies, procedures, and evidence-based quality improvement initiatives that improve efficiency, safety, and patient outcomes.

Requirements

• Massachusetts RN licensure required.
• At least five years of RN experience, including at least one year as a charge or lead RN.
• Current Basic Life Support (BLS) certification required.
• Bachelor of Science in Nursing (BSN) required.
• Strong clinical and theoretical nursing skills, knowledge of the nursing process, and effective communication with patients, families, physicians, and healthcare professionals.
• Ability to stand and walk for extended periods, lift or maneuver patients, and work around infectious diseases, blood-borne pathogens, and potentially carcinogenic chemicals.
