Outpatient Nurse Practitioner (Family Medicine)

Location: Lowell General Hospital, 198 Littleton Road, Lowell, MA; the role serves Westford Family Medicine in Westford, MA.

Salary: $124,800–$156,000 per year, plus performance incentive opportunities.

What you will do

• Provide comprehensive family medicine care, including health assessments, diagnosis, treatment, and follow-up.
• Build meaningful relationships with patients and support high-quality outcomes and patient satisfaction.
• Address same-day and routinely scheduled family medicine needs through in-office and telehealth appointments.
• Deliver proactive care management through preventive medicine and integrated population health.
• Collaborate independently and cross-functionally with multidisciplinary medical and community teams.
• Support active engagement with local communities by combining academic and community care.

Requirements

• Graduate of an accredited or approved Nurse Practitioner program.
• At least 2 years of Nurse Practitioner experience, ideally in primary care or family medicine.
• Current Registered Nurse and Nurse Practitioner licenses in the Commonwealth of Massachusetts.
• Excellent verbal and written communication skills for working with patients, families, and healthcare professionals.
• Patient-centered approach with a focus on high-quality care and patient satisfaction.
• Ability to work cooperatively with a broad range of medical disciplines.
