RN, Referral Coordinator (Healthcare)

Location: Mount Sterling, Illinois, United States; onsite at a health center. Full-time schedule with required hours on Monday and Thursday from 5:00–7:00 PM; remaining 36 hours are flexible within clinic hours.

Salary: $30.00–$45.00 per hour

What you will do

• Own the referral process, including authorizations, documentation, specialist selection, scheduling, communication, and follow-up.
• Provide concierge-level support by addressing barriers, coordinating with specialty offices, and tracking referrals through completion.
• Confirm appointments, obtain specialist records and recommendations, update the care team, and coordinate follow-up care.
• Assess patient needs, conduct nursing evaluations, and provide treatment within the scope of practice.
• Perform and document medical tests, screenings, treatments, medication administration, and emergency care.
• Maintain medical equipment, infection-control standards, patient records, and a safe clinical environment while educating patients on health promotion.

Requirements

• Certificate from an accredited nursing school or an associate degree in nursing is required; a BSN is preferred.
• Current Registered Nurse licensure in the state of practice is required.
• Current hands-on Basic Life Support certification from AHA, ARC, or an equivalent provider is required.
• Advanced Cardiac Life Support certification may be required depending on the site contract.
• At least three years of Registered Nurse experience in an emergency room, occupational health clinic, or other ambulatory care clinic.
• Occupational Health Nursing, Emergency Nursing, or Case Management certification is preferred and may be required at some sites.
