Home Health Registered Nurse, RN Case Manager, Lowell Area

Location: Lowell, Massachusetts and surrounding areas; home visits in the community. Full-time, 40 hours per week, with weekend and holiday commitment plus rotating evening, night, weekend, and holiday coverage.

Salary: $84,300.22–$107,481.20 per year.

What you will do

• Assess patients’ physical and psychosocial needs and provide professional nursing care in the home.
• Develop, implement, and evaluate individualized plans of care with patients, families, physicians, and interdisciplinary partners.
• Manage an assigned caseload, prioritize visits and calls, and provide telephone triage and problem-solving to help avoid hospitalization.
• Complete physician orders, OASIS documentation, medication reconciliation, and timely patient-record updates.
• Coordinate care conferences, discharge planning, community resources, and responses to critical needs or safety concerns.
• Provide patient and caregiver education, advocacy, emotional support, and care related to symptom management and end-of-life needs.

Requirements

• Massachusetts RN and New Hampshire RN licensure required.
• Valid state-issued driver’s license required for community-based home visits.
• At least one year of acute medical/surgical nursing experience.
• Basic Life Support certification.
• Ability to communicate, read, and write in English.
• Ability to work autonomously, manage time, prioritize a busy schedule, and meet the physical demands of home care, including regularly lifting 25 pounds and occasionally lifting up to 100 pounds.

Nice to have

• Bachelor of Science in Nursing.
• Specialty nursing certifications.
• Three years of acute nursing experience, including medical/surgical, oncology, hospice, or home health.
• Experience with IV therapy and complex wound care.
