Registered Nurse, RN- Acute Care Transition Team

Location: Lowell General Hospital, Main Campus Emergency Room

Pay range: $37.50–$68.47 per hour

What you will do

• Provide acute inpatient nursing care for patients awaiting inpatient beds, primarily in the Emergency Department.
• Conduct and document clinical assessments and develop, implement, and evaluate individualized care plans.
• Use nursing judgment to adapt care plans to changing patient health needs.
• Educate patients and families, support informed decisions and advance directives, and coordinate comprehensive discharge plans.
• Manage patient assignments, delegate according to patient needs and team-member skills, and collaborate with physicians and multidisciplinary teams.
• Support quality improvement, evidence-based practice, professional development, precepting, and the onboarding of new staff.

Requirements

• Massachusetts RN licensure required.
• Current Basic Life Support (BLS) certification required.
• Experience requirements depend on the applicable Community RN Career Direct Care Pathway level: Level I through Level IV.
• Department-specific certifications relevant to the patient population may be required.
• Strong communication, collaboration, nursing-process knowledge, and acute-care clinical judgment are required.
• Ability to lift 30–35 lbs occasionally, stand and walk for extended periods, and safely maneuver patients.
