LPN Intake and Care Coordination Nurse

Location: Bridgeton, Missouri, United States. Full-time, onsite schedule: Monday–Friday, 7:30 AM–4:30 PM; no nights, holidays, or on-call responsibilities.

What you will do

• Conduct comprehensive patient intake assessments and collect medical, medication, family, and social histories.
• Review and maintain patient information in the EMR, prepare charts, and ensure accurate clinical documentation.
• Follow up with patients about lab results, diagnostic testing, treatment plans, referrals, and specialist appointments.
• Monitor outstanding labs, imaging, referrals, and care-plan activities to support continuity of care.
• Assist providers with clinical inboxes, patient messages, prescription refill requests, and prior authorizations.
• Support chronic disease management, preventative care outreach, wellness events, health assessments, and population health initiatives.

Requirements

• Graduate of an accredited Practical Nursing program.
• Current and unrestricted Licensed Practical Nurse (LPN) license and current BLS certification.
• At least two years of nursing experience in primary care, family practice, or internal medicine.
• Experience using electronic medical record (EMR) systems.
• Knowledge of primary care workflows, chronic disease management, preventative care, referrals, prior authorizations, and insurance processes.
• Strong communication, customer service, organization, time management, accuracy, and multitasking skills.
