Onsite Care Coordination Nurse

Location: Hybrid/onsite at Advocate Christ Medical Center in Oak Lawn, Illinois; must live within a 30–60 minute driving distance. Work includes limited remote documentation from a private workspace and occasional travel to hirify.global offices for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support members and families with discharge planning, education, and follow-up primary care visits.
• Identify care gaps, safety concerns, and discharge barriers, then implement appropriate interventions.
• Collaborate with providers, facility case managers, and healthcare professionals to coordinate care.
• Educate members about diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation while maintaining infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in Illinois.
• Residence within a 30–60 minute driving distance of Advocate Christ Medical Center.
• At least three years of recent acute care nursing experience.
• Experience with hospital or health-plan discharge planning or care coordination.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, vehicle liability insurance, TB screening, and required background and facility credentialing clearances.

Nice to have

• Bachelor’s degree in Nursing.
• Care management, health promotion, coaching, utilization management, or health-plan experience.
• Bilingual English/Spanish skills.
• Care Management certification and knowledge of HEDIS and Stars measures.
