Registered Nurse (RN) Health Coach

Location: Albany, GA, United States; onsite

Schedule: Full time, Monday–Friday, 32 hours per week; no evenings, weekends, holidays, or on-call responsibilities.

What you will do

• Provide individualized health coaching, education, and ongoing support to help patients achieve their health goals.
• Guide patients in managing chronic conditions through evidence-based education and behavior-change techniques.
• Educate patients about disease processes, medications, treatment plans, and preventive care.
• Review health assessments, biometric screenings, laboratory results, and other clinical data to identify opportunities for improved outcomes.
• Collaborate with primary care providers and interdisciplinary care team members to support coordinated, patient-centered care.
• Document patient interactions, coaching activities, and progress in the electronic medical record.

Requirements

• Current, active Registered Nurse license in the state of practice.
• At least two years of experience in chronic disease management, care management, health coaching, patient education, population health, or a related clinical setting.
• Strong clinical assessment, communication, and patient education skills.
• Passion for prevention, wellness, relationship-based care, and helping patients improve their health.

Nice to have

• Bachelor’s degree in nursing or a related field.
