Onsite Health Coach

Location: 100% onsite at the client's location in St. Louis, Missouri, United States. Field-based work may involve travel to client sites or designated locations, with occasional office attendance. Candidates must live within a reasonable commuting distance. Work schedule: Monday–Friday, 8:00 a.m.–5:00 p.m. EST.

Compensation: $5,000 sign-on bonus.

What you will do

• Conduct behavioral assessments to identify members' knowledge, skills, and behavioral needs.
• Coordinate health coaching activities with nurse case managers based on identified objectives and goals.
• Apply behavior change principles to identify barriers and develop strategies to overcome them.
• Provide feedback on member goal attainment and clinical issues.
• Monitor and evaluate interventions, modifying them as needed.

Requirements

• Bachelor's degree in an appropriate field such as Health Education, Exercise Physiology, Respiratory Therapy, or Dietetics, plus at least three years of related experience, or an equivalent combination of education and experience.
• Certification, advanced certification, and/or a relevant license as required for the specialty.
• Experience or knowledge in health coaching, disease management, behavioral or clinical assessment, patient education, and behavior change techniques.
• Understanding of disease management principles.
• Ability to work onsite in St. Louis, Missouri, within a reasonable commuting distance.
