Social Worker, Social Services (Healthcare)

Location: Work at home in Kentucky, United States; occasional travel to hirify.global offices may be required for training or meetings. Working hours are Monday through Friday, 9:30 AM–6:00 PM EST; training during the first two weeks runs from 8:30 AM–5:00 PM EST.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide licensed, master's-level social work services within the Special Needs Plan program as part of an interdisciplinary care team.
• Conduct inbound and outbound telephonic outreach to assess members' social determinants of health needs.
• Coordinate care, provide health education, connect members with plan benefits and community resources, and advocate for appropriate support.
• Collaborate with nurse care managers, dietitians, behavioral health specialists, pharmacists, providers, and community partners.
• Assess for additional support needs, initiate referrals, and develop and maintain individualized member care plans.
• Document interactions and care activities accurately to support CMS compliance and continuity of care.

Requirements

• Master's degree in social work from an accredited university.
• Current, unencumbered LMSW, LCSW, or LICSW license and a passed ASWB Master's-Level exam.
• At least three years of experience as a social worker in a medical healthcare setting.
• Proficiency with Microsoft Word, Outlook, and Excel, plus the ability to manage multiple priorities and applications.
• Willingness to obtain and maintain social work licensure in multiple states as required by business needs.
• Dedicated home workspace and wired cable or DSL internet connected directly to a modem, with at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Telephonic experience with geriatric, vulnerable, or low-income populations.
• Experience with Epic EMR and Medicare or Medicaid.
• Licensure in AL, FL, GA, KY, LA, MS, NC, OK, or SC.
• Bilingual English/Spanish or English/Creole skills.
