Supervisor, Transitions of Care Associate (Healthcare)

What you will do

• Lead, coach, and mentor Transitions of Care Associates against performance, quality, and patient experience goals.
• Provide direct transitions-of-care patient support for approximately 30% of working time.
• Conduct 1:1 meetings, team meetings, coaching sessions, onboarding, and competency development.
• Ensure adherence to care workflows, documentation standards, escalation pathways, and timely post-discharge outreach.
• Monitor KPIs, analyze trends and barriers, and identify process improvement opportunities.
• Partner with leadership on staffing, workload balancing, scheduling, and complex patient cases.

Requirements

• Active and clear Medical Assistant Certification required.
• At least 2 years of direct lead or supervisory experience in a virtual setting required.
• Strong organization, attention to detail, proactive problem-solving, and people-development skills.
• Ability to support high-quality transitions from hospital discharge to home-based care.

Nice to have

• Experience in a remote, high-volume, fast-paced environment.
• Experience with transitional care, post-acute workflows, or care coordination.
• Full bilingual proficiency in English and Spanish.
