Nurse Care Coordinator Lead (RN)

Location: Remote, California, United States; Monday–Friday, 9 AM–6 PM PST with a one-hour break and occasional Saturday coverage.

Salary: $90,000–$110,000/year, depending on geographic location, with medical and ancillary benefits.

hirify.global is a healthcare startup developing a technology-enabled virtual pediatric chronic care platform, initially focused on ADHD, anxiety, obesity, and overweight management.

What you will do

• Recruit, onboard, supervise, coach, and manage the performance of nurses, medical assistants, and community health workers.
• Prepare staff schedules, maintain staffing coverage, and report team performance metrics to clinical leadership.
• Coordinate clinical appointments, referrals, lab work, care transitions, and escalation of care across interdisciplinary teams.
• Provide evidence-based education to children’s families about chronic condition management, medication regimens, treatment plans, and care goals.
• Manage medication prior authorizations, pharmacy communications, refills, authorization tracking, and medication records.
• Maintain healthcare compliance and accurate documentation while participating in quality improvement and process optimization.

Requirements

• Active California Registered Nurse (RN) license in good standing required. An Associate’s, Bachelor’s, or Master’s degree in Nursing is accepted.
• Experience leading a team of clinical staff.
• Proficiency with multiple technology platforms and electronic health record systems.
• Strong organizational, communication, and interpersonal skills, with the ability to manage competing priorities in a remote virtual environment.
• Comfort working with diverse, medically and socially complex pediatric populations.
• English and Spanish bilingual proficiency required; reliable internet and a dedicated remote workspace required.

Nice to have

• Experience in pediatric, chronic care, or care coordination settings.
• Familiarity with prior authorization processes and payer requirements.
• Experience working in underserved or urban environments.
