Bilingual (English/Spanish) Cardiac Rehab Intake Specialist - Contractor

Location: United States — Remote; ideally available between 10am and 5pm ET.

Compensation: $50 per 90-minute session.

What you will do

• Conduct initial assessments for patients enrolling in the virtual cardiac recovery program.
• Educate patients about program benefits, risk-factor modification, cardiac conditions, procedures, and medications.
• Coach and support patients through video sessions and electronic chat messaging.
• Complete individualized nutrition, psychosocial, fitness, and co-morbidity assessments and treatment plans.
• Review medical histories, set goals, and use motivational interviewing for special populations.
• Collaborate with exercise physiologists, patient experience, product, and marketing teams while adapting clinical best practices to remote care.

Requirements

• At least three years of experience helping people manage their health, ideally in a clinical or remote health-coaching setting.
• Degree in a health-related field and experience providing disease-management counseling or education in a medical setting.
• Knowledge of cardiac rehabilitation, chronic conditions, nutrition and exercise trends, and basic clinical documentation.
• Experience working with elderly patients and some clinical-setting experience.
• Fluent English and Spanish required, both written and verbal.
• Detail-oriented, resourceful, creative, team-oriented, and committed to diversity and inclusion.

Nice to have

• Experience coaching smoking cessation, diabetes management, or other special populations.
• Experience supporting patients with CHF, mental health, substance abuse, women’s health, or related conditions.
