What you will do

• Conduct pre- and post-assessments for mental health concerns, develop intervention plans, and refer participants to partner agencies.
• Meet with participants individually to assess job-search plans, discuss employment barriers, and provide follow-up during subsidized employment.
• Coordinate supportive and retention services, conduct outreach to participants who miss appointments, and monitor participation in program activities.
• Maintain accurate case notes, participant records, data, plans, and required documentation in manual and electronic systems.
• Prepare and submit success stories and other documentation according to contract deadlines.
• Support team training, mentoring, performance auditing, policy implementation, and weekly unit meetings.

Requirements

• Licensed Clinical Social Worker or Licensed Medical Social Worker credential.
• Two years of professional experience in employment, recruiting, staffing, workforce development, or an equivalent combination of education and experience.
• Strong organizational, time-management, written, verbal communication, and teamwork skills.
• Experience with complex case-file management and Microsoft Office programs.
• Current valid driver's license, good driving record, and transportation for local travel.
• Ability to work independently, maintain confidentiality, and comply with company and funder policies.

Nice to have

• Two to four years of complex case-file management experience.
• At least one year of supervisory experience.
