Location: Charlotte, North Carolina, USA; CONUS and across the Mid Atlantic Region

Employment type: Full-time. The engagement is offered as an exempt 1099 contract or contract W-2 arrangement, with the possibility of conversion to a full-time hirify.global W-2 position.

What you will do

• Assess and manage high-need client cases.
• Develop comprehensive case plans with measurable goals.
• Coordinate integrated care with multiple service agencies.
• Evaluate case outcomes and adjust plans as needed.
• Supervise, train, and mentor junior case management staff or aides.
• Participate in policy and program development.

Requirements

• Bachelor’s or master’s degree in social work or a related field.
• 5+ years of case management experience.
• Advanced knowledge of case management methodologies.
• Leadership, advocacy, and conflict-resolution skills.
• Critical thinking, empathy, and cross-cultural competence.
• Experience handling cases requiring multi-agency coordination; supervisory experience is preferred.
