Supportive Housing Case Manager (Social Services): Providing case management, life skills development, targeted interventions, and referrals for homeless individuals and families with an accent on supportive housing coordination, client eligibility, and community-based services. Focus on developing action plans, preventing evictions, coordinating public benefits and healthcare referrals, and tracking client outcomes.
Location: Longview, Washington, USA. This is an onsite role requiring daily attendance in the office and community outreach.
Salary: $21.50–$23.95 per hour

What you will do
- Conduct client assessments and interviews to determine eligibility and develop or update action plans, case notes, and related documentation.
- Provide ongoing life skills development and targeted interventions for clients at risk of eviction.
- Connect clients with medical, mental health, counseling, and community support services through appropriate referrals.
- Guide clients through applications, qualification, and re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other benefits.
- Conduct occasional home visits, wellness checks, local travel, and client transportation as needed.
- Track and report outcomes, participate in internal and external meetings, and represent Compass at professional and community events.
Requirements
- Bachelor's degree in social services or a related field plus one year of experience coordinating supportive housing or independent living services; or a high school diploma/GED plus two years of relevant experience.
- Current resume must include bulleted details of direct housing experience.
- Strong reading, writing, documentation, communication, collaboration, time management, and problem-solving skills.
- Valid driver's license, insurance, and access to a vehicle insured for the applicant as a driver.
- Ability to work a flexible Monday–Friday daytime schedule, including some nights and weekends.
- Ability to perform CPR/First Aid as needed, lift up to 50 pounds, and use laptops, cell phones, and designated software in the community; applicants must be at least 21 years old.
