Wellness Nurse (LPN) - Per Diem All Shifts

What you will do

• Provide direct nursing care to residents with an emphasis on holistic wellness.
• Monitor residents’ health, safety, and well-being.
• Conduct thorough resident assessments.
• Assist with training and monitoring medication administration.
• Supervise care staff in line with state regulations and community policies.
• Maintain clinical quality assurance according to federal, state, and local standards.

Requirements

• Current LPN license and graduation from an accredited school of nursing.
• At least two years of experience in long-term care, assisted living, home health, or a hospital setting preferred.
• Experience working with residents with Alzheimer’s disease or related dementias.
• Knowledge of nursing practices, techniques, methods, and applicable federal and state regulations.
• Computer proficiency, including Microsoft Office, Windows, Outlook, and Excel, with the ability to learn new applications.
• Ability to perform physical duties, including lifting or carrying up to 50 lbs and supporting residents.
