Care Navigator

Location: Remote within the United States; not available in Alaska, Maine, Washington, DC, New Jersey, California, New York, Massachusetts, Connecticut, Colorado, Washington State, Oregon, or Minnesota. U.S. work authorization is required.

Salary: $45,000–$52,500 per year, plus a target performance-based bonus.

What you will do

• Provide a supportive experience for clients, families, referral sources, and external providers.
• Coordinate with clients, families, referral sources, and internal clinical and admissions teams.
• Facilitate admissions for at-risk youth and make timely outbound referrals for clients who are not admitted.
• Document client and referral-source interactions accurately in the electronic record system.
• Collaborate with Clinical Outreach and Partnerships teams to understand referral sources and their services.
• Meet admission quotas and performance goals, including admission rates, patient satisfaction, and time-to-admission metrics.

Requirements

• Bachelor’s degree in health sciences, communications, psychology, social work, or a related field.
• 1–2 years of relevant experience, preferably in healthcare or customer- and patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach.
• Strong interpersonal, relationship-building, listening, written communication, and verbal communication skills.
• Metrics- and results-oriented approach with experience meeting concrete targets and KPIs.
• Excellent organization, attention to detail, and ability to learn quickly in a fast-paced environment.
• Proficiency in Salesforce and Google Suite or Microsoft Office; native or bilingual English proficiency required.
