Nurse Practitioner – In Home Assessments

Location: Ventura, California, United States; on-site with daily travel within a 55-mile radius of the assigned county

Compensation: Approximately $100 per completed assessment plus paid mileage

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused patient histories, physical exams, vital-sign checks, medication reviews, and screenings.
• Complete screenings including Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members about preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently in the field while maintaining reliability and accountability for confirmed visits.

Requirements

• Active, unencumbered Nurse Practitioner license in the applicable state.
• At least 3 years of patient care experience preferred.
• Comfort with EMR systems and independent work.
• Ongoing monthly availability with a minimum commitment of approximately 30 hours per month.
• Consistency during onboarding and scheduling, including honoring confirmed visits and assignments.
• Willingness to obtain additional licensure if needed, with support from hirify.global.
