Part-Time Registered Nurse Care Manager (RNCM)

Location: Hybrid and field-based in Riverside, California, United States; remote clinical work is combined with in-person member and provider engagement as needed.

Salary: $40.00–$50.00 per hour

What you will do

• Review member charts, medical histories, care plans, and treatment adherence in EMR/EHR systems.
• Provide clinical oversight and guidance to non-clinical Lead Care Managers handling complex cases.
• Identify gaps in care and recommend interventions to improve member health outcomes.
• Coordinate assessments, referrals, care transitions, discharge planning, and follow-up with providers, hospitals, and community organizations.
• Conduct field-based visits and in-person member engagement activities when required.
• Participate in the weekly ECM case review meeting and contribute clinical input to reports.

Requirements

• Active Registered Nurse license in California required.
• At least 2 years of nursing experience.
• At least 1 year of experience in care management, case management, or a leadership capacity.
• Experience supporting vulnerable populations and individuals with complex medical and social needs.
• Strong clinical assessment, care planning, communication, collaboration, and clinical judgment skills.
• Ability and willingness to perform field-based work and in-person member engagement as required; one hour per week is required for the ECM case review meeting.
