Special Operations Case / Nurse Case Manager Part-Time (National Capitol Region)

Location: Alexandria, Virginia, United States. U.S. citizenship is required, along with a current license in a U.S. state, the District of Columbia, Guam, Puerto Rico, or the U.S. Virgin Islands. A Secret or Top Secret clearance, or the ability to obtain one, is required.

Compensation: $49.31–$54.64 per hour, based on working location, knowledge, skills, abilities, experience, and internal parity.

What you will do

• Collect and document medical assessment data and develop time-sensitive treatment plans for assigned patients and populations.
• Conduct telephone assessments using established protocols and coordinate appropriate, cost-effective care.
• Set care priorities, monitor patient progress, and coordinate referrals with healthcare providers and facilities.
• Oversee discharge planning and ensure appropriate timing and sequencing of care.
• Apply utilization review criteria, VA/DOD clinical practice guidelines, and quality and risk management standards.
• Analyze healthcare outcomes, manage information systems, and prepare reports, presentations, and educational briefings.

Requirements

• U.S. citizenship and a Secret or Top Secret clearance, or the ability to obtain one.
• Current, full, active, and unrestricted RN or Licensed Clinical Social Worker license in an eligible U.S. jurisdiction.
• At least two years of nursing or case management experience.
• Case manager certification from a recognized organization, or certification within the first year of employment.
• Experience managing complex medical, psychosocial, and financial problems and consulting with healthcare providers.
• Fluent written and oral English; current American Heart Association Basic Cardiac Life Support certification is required.
