Wellness Coordinator - Registered Nurse (Part-Time)

Location: On-site in Middletown and Westerly, Rhode Island, USA; travel between locations required

Salary: $32–$37 per hour plus bonus structure

What you will do

• Lead and manage the Health Home team supporting opioid treatment program patients.
• Identify patients, conduct outreach, assess needs, and develop individualized care plans.
• Coordinate care management, preventive services, health promotion, mental health services, substance use services, and transitions of care.
• Connect patients with community resources, social support, long-term care, and recovery services.
• Supervise case managers, facilitate team meetings, establish care protocols, and manage patient engagement.
• Monitor service documentation and billing in SAMMS, maintain provider agreements, and lead quality improvement activities.

Requirements

• Master’s degree in Social Services, Psychology, Social Work, or Criminology from an accredited institution, or Registered Nurse qualification.
• Familiarity with behavioral health and related experience.
• Strong integrity, communication skills, and ability to engage with people from diverse backgrounds.
• Structured approach to managing high caseloads.
• Ability to work on-site at other hirify.global Rhode Island locations as needed.
• Ability to perform extended periods of computer work, sitting, keyboarding, and in-person or telephone communication.
