Salary: $63,000–$73,000 annually

What you will do

  • Build and sustain relationships with rural healthcare providers, community organizations, public health groups, health systems, and community health workers.
  • Coordinate outreach and provider engagement with the state Community Health Manager and local and state Association teams.
  • Guide providers through program onboarding, training access, referrals, and ongoing participation in rural health initiatives.
  • Support the adoption of resources and services related to brain health, care navigation, dementia care, and workforce enhancement.
  • Track and report onboarding progress, partnership status, program impact, trends, challenges, and growth opportunities.
  • Provide administrative and reporting support for the referral program and broader rural health strategy.

Requirements

  • Bachelor’s degree required, preferably in Public Health, Health Administration, Social Work, Nursing, Public Policy, healthcare, human services, or a related field.
  • 3–5 years of relevant experience.
  • Demonstrated project management skills, including planning, implementation, assessment, and delivery of results.
  • Strong verbal and written communication skills for working with individuals, community groups, clinicians, administrators, and leaders.
  • Understanding of clinical workflows in primary care, hospitals, and multispecialty settings, with familiarity with EHR environments and provider operations.
  • Ability to manage multiple provider relationships, onboarding projects, timelines, tracking platforms, and reporting responsibilities.
