Senior AI Fullstack Engineer - Backend Focus (AI)

Location: Must live close to one of the designated IT locations in the United States: New York City, Louisville, Tampa, Fort Lauderdale, Dallas/Frisco, Boston, Washington, D.C., Atlanta, Nashville, Chicago, or Charlotte.

Salary: $106,900–$147,000 per year, plus eligibility for a bonus incentive plan.

What you will do

• Design and lead core backend services, REST and GraphQL APIs, and service-to-service integrations for CenterWell’s AI product stack.
• Establish authentication, authorization, rate limiting, PHI access control, and audit logging patterns with HIPAA compliance built into the platform.
• Implement reusable AI and LLM integration patterns, including routing, streaming responses, tool orchestration, and model output handling.
• Build logging, monitoring, and alerting infrastructure for platform observability and reliability at scale.
• Contribute to end-to-end product features with TypeScript and Next.js, including server-side APIs and rendered user interfaces.
• Lead architecture reviews and define backend engineering standards.

Requirements

• 5+ years of software engineering experience with deep backend and platform engineering expertise.
• Strong TypeScript and/or Python skills for API and service development.
• Experience building scalable, production-grade REST or GraphQL APIs.
• Knowledge of OAuth2, OIDC, API security, authorization models, and HIPAA requirements for PHI.
• Experience with Next.js and React for full-stack feature development.
• Hands-on experience with AI-assisted development tools and LLM coding harnesses such as Claude Code, Pi Agent, Cursor, or Codex.

Nice to have

• Experience in healthcare, financial services, or another highly regulated industry.
• Exposure to event-driven architecture, microservices, or API gateway patterns at scale.
• Familiarity with streaming APIs and AI/LLM tool-use orchestration.
• Experience in a greenfield or team-founding engineering environment.
