Principal Engineer (AI)

Location: San Francisco, United States; in-person at the San Francisco HQ. Partial remote may be considered for exceptional candidates.

Salary: $250,000–$400,000 per year depending on experience, plus meaningful early-stage equity.

What you will do

• Own complex, load-bearing engineering domains end to end, including architecture, implementation, documentation, and operational outcomes.
• Improve the engineering delivery system, including merge gates, migration safety, canary releases, rollback systems, and high-volume AI-assisted development.
• Develop the event-sourced clinical work spine for orders, tasks, commands, receipts, auditability, and fail-closed safeguards.
• Operate and expand production LLM agents with tool use, checks, receipts, evaluation harnesses, and observability.
• Own the data and metrics estate, including the warehouse, analytics platform, and pipelines.
• Make architecture decisions, challenge assumptions, and communicate decisions and trade-offs clearly in writing.

Requirements

• Principal-level experience owning architecture across multiple companies or many codebases as a staff-plus individual contributor, chief architect, or platform consultant.
• Typically 10+ years of experience and ownership of multiple systems end to end; demonstrated expertise and impact matter more than the exact number.
• Experience shipping daily at high volume with AI agents, including hands-on use of agent tooling and unsupervised workflows.
• Ability to take ambiguous, complex problems from investigation through implementation, documentation, and delivery without relying on narrowly scoped tickets.
• Strong written communication and clinical-grade engineering discipline, including auditability, HIPAA, fail-closed defaults, and explicit invariants.
• Experience with TypeScript, Node, Postgres, Next.js on Vercel, AWS, production LLM agent orchestration, EHR and e-prescribing integrations, PHI-grade security, and audit trails.

Hiring process

• Seven- to ten-day process beginning with a 20-minute coffee chat and a 60-minute systems deep dive.
• Async take-home exercise using a simplified slice of the real repository, including an implementation PR and agent session logs; a paid two- to five-day work trial may be offered as an alternative.
• Founder and engineering interviews followed by a written decision within 48 hours.
