Occupational Therapist (PACE)

Location: 2420 Merced Street, San Leandro, California 94577, United States; home visits and travel between work sites or community locations may be required.

Base salary: $112,000–$129,000 annually, plus a target annual bonus of 5% and potential additional incentive compensation.

What you will do

• Provide occupational therapy evaluations and treatment across acute, restorative, and maintenance levels of care.
• Assess participants, create individualized treatment plans, and explain therapy implementation to participants and caregivers.
• Conduct home visits as needed and educate participants and families on ADLs, exercise programs, supports, and durable medical equipment.
• Contribute clinical assessments, recommendations, and care coordination as a member of the Interdisciplinary Team.
• Document therapy services and maintain participant records in accordance with regulatory requirements.
• Support therapy program standardization, quality audits, DME inventory, data collection, safety, and infection control.

Requirements

• Master’s or Doctorate in Occupational Therapy, or an equivalent degree from an accredited program meeting AOTA and ACOTE standards.
• National certification by the American Occupational Therapy Association or eligibility for its National Registration Examination.
• At least one year of experience working with frail or elderly populations.
• Ability to work in a fast-paced, evolving environment, navigate ambiguity, adapt quickly, and contribute to continuous improvement.
• Strong learning and growth mindset, integrity, ownership, and commitment to feedback and data-informed decisions.
• Must be authorized to work in the United States; hirify.global participates in E-Verify.

Nice to have

• Two or more years of practicing occupational therapy experience.
• Spanish/English bilingual ability; additional languages are a plus.
• PACE or ADCH/CBAS experience.
• State-issued driver’s license, personal transportation, and legally required auto insurance.
