Clinical Pharmacist - Ambulatory Care / COPD Population Health Program (COPD)

Location: Rhode Island, United States; travel between facilities, provider offices, and assigned practice sites is required.

Salary: $120,000–$130,000 USD per year

What you will do

• Serve as the medication expert on interdisciplinary primary care teams.
• Reconcile and optimize medication regimens for safety, effectiveness, adherence, and cost-conscious care.
• Deliver COPD-focused chronic disease management, including guideline-directed therapy, inhaler technique education, adherence monitoring, and access support.
• Identify and engage high-risk COPD patients, including those with recent emergency department visits, hospitalizations, or readmissions.
• Conduct transitions-of-care medication evaluations and coordinate follow-up with primary care teams and other providers.
• Support quality improvement initiatives, population health programs, documentation, reporting, presentations, and development of ambulatory pharmacy services.

Requirements

• Current unrestricted pharmacist licensure in accordance with the Rhode Island Board of Registration in Pharmacy is required.
• Prior experience practicing under a Collaborative Practice Agreement, including independently initiating, adjusting, or discontinuing medication therapy and ordering laboratory tests.
• Ability to make independent clinical decisions while practicing at the top of the pharmacist license.
• Advanced chronic disease management training through Board of Pharmaceutical Specialties certification, an ASHP-accredited residency, or the required degree and professional experience pathway.
• PharmD with at least two years of professional experience, or Bachelor of Science in Pharmacy with at least three years of professional experience.
• Compliance with Rhode Island pharmacy regulations, applicable U.S. federal requirements, and departmental policies.

Nice to have

• Ambulatory care, population health, or chronic disease management experience.
• Experience managing COPD, asthma, or other respiratory disease states, including inhaler education.
• ASHP-accredited PGY-1 or PGY-2 residency, especially in ambulatory care.
• Experience with interdisciplinary care teams, hospital-discharge transitions, electronic health records, population health platforms, and reporting tools.
• Proficiency with Microsoft Word, Excel, Outlook, and web-based systems.
