Clinical Pharmacist

Location: Remote within the United States; active Texas pharmacist license required. Ability to work business hours in the PST or MST time zone preferred, Monday-Friday 8am-6pm.

What you will do

• Perform comprehensive medication reviews and medication reconciliation after discharge for Virtual Primary Care and Complex Care and Case Management members.
• Collaborate with Virtual Primary Care teams and patients on standing orders, medication therapy management, and chronic disease management.
• Recommend or initiate therapies, monitor patients longitudinally, and adjust treatment as indicated.
• Guide members and colleagues through pharmacy benefits navigation, treatment guidance, medication support, and specialty pharmacy needs.
• Develop and present educational content for members and provide training and presentations for internal teams.
• Support pharmacy strategy by developing interventions and assessing internal and external pharmacy solutions.

Requirements

• Active pharmacist license in good standing with the Texas State Board of Pharmacy required.
• Multiple state licenses are strongly preferred, with willingness to obtain additional licenses required.
• Doctor of Pharmacy (PharmD) from an accredited program.
• At least 5 years of experience in primary care, ambulatory care, or a similar clinical setting, including experience within an integrated health system or health plan.
• At least 2 years of experience with specialty pharmacy and high-cost medications.
• Empathetic patient care, strong written and verbal collaboration skills, multidisciplinary teamwork, and strict adherence to HIPAA and patient confidentiality.

Nice to have

• Multiple state pharmacist licenses, especially in Arkansas, California, Washington, Colorado, Arizona, or Iowa.
• Board Certification in Ambulatory Care or Pharmacotherapy, such as BCACP or BCPS.
• Completion of a PGY1 Pharmacy Residency program.
• Availability during PST or MST business hours.
