Staff Physician (IM/FM)

Location: Onsite in Lost Hills and Delano, California, USA. Relocation assistance is available.

Salary: $240,000–$270,000 annually, plus potential performance bonus.

What you will do

• Provide compassionate, evidence-based outpatient primary care, including preventive care, chronic disease management, and acute-condition treatment.
• Diagnose and manage obesity, diabetes, hypertension, dyslipidemia, and other cardiometabolic conditions.
• Develop individualized treatment plans and support patients in long-term health and disease management.
• Manage an empaneled patient population and coordinate care with physicians, advanced practice providers, behavioral health providers, physical therapists, health coaches, and other clinical staff.
• Review clinical and population health data and participate in quality improvement, prevention, patient engagement, and access initiatives.
• Coordinate referrals, follow-up care, specialty services, documentation, and onsite, mobile, and community-based care efforts.

Requirements

• MD or DO degree from an accredited medical school.
• Board certification in Family Medicine or Internal Medicine is preferred.
• Current or eligible for unrestricted California medical licensure.
• Current DEA registration or eligibility to obtain it, and current BLS certification or ability to obtain it before starting.
• Strong knowledge of adult primary care, preventive medicine, obesity, diabetes, and cardiometabolic disease management.
• Strong patient communication skills, electronic medical record proficiency, and ability to work in an integrated multidisciplinary care model.

Nice to have

• Outpatient primary care experience.
• Experience managing obesity, diabetes, and related cardiometabolic conditions.
• Fluency in medical Spanish.
