Palliative Oncology Nurse Navigator

Location: Remote within the lower 48 United States

What you will do

• Conduct conversations about goals, values, advance care planning, and palliative care through phone, email, and video.
• Perform comprehensive symptom assessments and provide longitudinal clinical support to members with serious illness and at the end of life.
• Facilitate end-of-life care discussions, address clinical issues, and manage urgent escalations.
• Coordinate care with Nurse leaders, Medical Directors, payers, providers, and non-clinical Care Team members.
• Participate in case conferences, maintain clinical documentation, and develop targeted support plans.
• Help address social determinants of health, including food resources and transportation access, while supporting care delivery and reducing readmissions.

Requirements

• Active, unrestricted compact Registered Nurse license required.
• New York, Illinois, and/or Michigan nursing licensure preferred.
• Associate Degree in Nursing or Bachelor of Science in Nursing required; BSN preferred.
• At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice caring for oncology patients and their caregivers.
• OCN certification is required at hire, or a commitment to obtain it within 2 years of hire.
• Spanish fluency is preferred but not required.

Nice to have

• Certified Hospice and Palliative Nurse certification.
• Advanced oncology or case management certifications, including AOCN, AOCNS, or CCM.
