Advanced Practice Provider Fellowship-Primary Care, Full-time, Days (Primary Care)

Location: 251 E. Huron, Chicago, IL, United States. Full-time day schedule, Monday–Friday, 8:00am–5:00pm; no call, weekends, holidays, or nights.

Salary: USD 42.31–55.43 per hour. Listed compensation: USD 44.23 per hour.

What you will do

• Complete a 12-month supervised fellowship with multidisciplinary clinical rotations and didactic training.
• Provide direct primary care to adult patients under the guidance of APP preceptors and attending physicians.
• Perform health assessments, physical examinations, diagnostic testing, treatment planning, medication management, and clinical documentation.
• Coordinate discharge and follow-up care with patients, families, physicians, nurses, and other healthcare professionals.
• Participate in committees, quality improvement initiatives, evidence-based practice, and clinical policy development.
• Educate patients, families, and professional groups on preventive care, medical conditions, treatments, and expected outcomes.

Requirements

• Eligible applicants must qualify as either an APRN-CNP or Physician Assistant.
• APRN-CNP applicants need an accredited nurse practitioner program, a master's degree in nursing, Illinois RN and APRN licenses, and Adult-Gerontology Nurse Practitioner certification.
• PA applicants need graduation from an accredited PA program and an Illinois PA license.
• Basic Life Support for Healthcare Providers certification from the American Heart Association is required.
• Completion of credentialing and privileging requirements and compliance with applicable clinical, regulatory, and accreditation standards are required.
