Float Case Manager

Location: Remote within the southwest region of Wisconsin, covering Trempealeau, Jackson, La Crosse, Monroe, and Clark counties; up to 40% travel throughout the region and occasional travel to hirify.global offices for training or meetings.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses to deliver services in the least restrictive environment defined by each care plan.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically in members’ residences.
• Complete quarterly in-person visits and maintain monthly phone contact with members.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, control service costs, and maintain case notes, authorizations, and care plan updates.

Requirements

• Bachelor’s degree in Human Services or a related field with one year of experience working with a Family Care target population, or a bachelor’s degree in another field with three years of related experience.
• At least three years of Family Care experience.
• Must reside within the southwest region of Wisconsin.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to travel up to 40% throughout the southwest Wisconsin region.
• Willingness to complete required tuberculosis screening and protect member PHI/HIPAA information while working from a dedicated home workspace.
