What you will do

• Provide comprehensive, patient-centered primary care across the full lifespan in a 100% outpatient setting.
• See approximately 16–20 patients per day and manage an assigned panel, including preventive care and chronic disease management.
• Collaborate with physicians, advanced practice providers, and care teams to coordinate care and improve outcomes.
• Practice in accordance with Rhode Island regulations, including consultation and referral as appropriate.
• Document care and manage patient records using the EMR.
• Participate in care coordination, quality improvement, population health initiatives, and shared after-hours call coverage.

Requirements

• Graduate of an accredited Nurse Practitioner program.
• Board certification as a Nurse Practitioner, such as FNP or AGNP, or in a related primary care specialty.
• Licensed or eligible for licensure as an APRN in Rhode Island.
• Active DEA registration or eligibility to obtain one.
• Rhode Island Controlled Substances Registration or eligibility to obtain one.
• Strong clinical judgment, communication, collaboration, and commitment to patient-centered value-based care.
