Float Primary Care NP/PA

Location: Roseville, Minnesota, with regular coverage of clinics in Highland Park, South Minneapolis, Eagan, Rosedale, and Crystal. The role requires in-person work and travel between clinic locations.

What you will do

• Evaluate and manage patients’ acute and chronic medical conditions.
• Manage multiple medications and address patients’ mental, social, and emotional health needs.
• Provide thorough, professional, and empathetic patient education.
• Assess, plan, and deliver care in partnership with other care-team members.
• Support high-quality primary care delivery across multiple clinics.
• Use data and population-health practices to support quality metrics and clinical best practices.

Requirements

• Master’s degree in Nursing or Physician Assistant studies; a Doctor of Nursing Practice is preferred.
• Adult or Family Nurse Practitioner or Physician Assistant qualification, with primary care, geriatrics, palliative care, or family medicine experience.
• Completion of an accredited NP or PA program and 2,080 hours of post-graduate collaborative practice in an integrated clinical care setting.
• Knowledge of evidence-based geriatric primary care, behavioral health, medication management, and preventive health guidelines.
• Current ANCC/AANP NP certification or PA-C designation through an ARC-PA-certified program and successful PANCE completion.
• Current Minnesota RN or PA licensure, a valid Minnesota driver’s license, and proof of US citizenship or eligibility to work in the US with a permanent resident card are required.

Nice to have

• Doctor of Nursing Practice degree.
• Geriatrics experience.
• Advanced Life Support certification.
