Location: 850 Del Mar Downs Rd, Solana Beach, CA 92075, USA; onsite senior living community

Salary: USD 36–41 per hour. Full-time PM shift: 2:30pm–11:00pm, including weekends.

What you will do

• Deliver nursing services in accordance with healthcare provider orders, the Nurses Practice Act, state regulations, and company policies.
• Assess residents, develop and update person-centered service plans, and follow up on changes in health conditions.
• Coordinate communication and continuity of care with healthcare providers, families, pharmacies, and other healthcare agencies.
• Support medication management, including medication delivery systems, storage, security, administration, and pharmacy collaboration.
• Document care in Yardi EHR, initiate incident reports, and participate in investigations, quality assurance, audits, and infection control activities.
• Coordinate medical, dental, vision, hearing, podiatry, telehealth, laboratory, and durable medical equipment services.

Requirements

• Valid nursing license in good standing as a Licensed Vocational Nurse, Licensed Practical Nurse, and/or Registered Nurse.
• First Aid and CPR certification required.
• Ability to work a flexible schedule, including weekends and holidays as needed.
• Strong clinical evaluation, organizational, time-management, communication, problem-solving, and decision-making skills.
• Ability to work with seniors and interact with cognitively impaired individuals.
• Proficiency with Microsoft Office, eMAR, and related software; criminal background clearance required before starting work.
