Activities Coordinator (Healthcare)

Location: Sacramento, CA, United States. The role involves working in participant care settings, day centers, community locations, and traveling between work sites as needed.

Salary: $26–$31 per hour, plus a target annual bonus of 5%.

What you will do

• Conduct recreational and social assessments and document participant needs, engagement, and outcomes in accordance with PACE requirements.
• Plan and lead therapeutic group activities supporting physical, cognitive, emotional, and social well-being.
• Create the center’s monthly activity calendar and identify community resources and future programming opportunities.
• Collaborate with Physical and Occupational Therapists and the Interdisciplinary Team on care plans, SMART goals, and restorative programming.
• Adapt activities for different cognitive and physical abilities, cultural backgrounds, and individual preferences.
• Maintain safe, organized activity spaces, monitor participant progress, and use appropriate approaches for dementia, cognitive impairment, and behavioral health needs.

Requirements

• 1–3 years of experience providing recreational therapy or activity programming to aging and frail populations.
• Active CPR/BLS certification.
• Experience conducting recreational and social assessments and documenting participant engagement and behavioral trends.
• Ability to collaborate as a core member of an Interdisciplinary Team and contribute to individualized care planning.
• Strong communication, reliability, accountability, judgment, adaptability, and safety awareness.
• Must comply with applicable U.S. employment eligibility, infection control, PPE, and vaccination requirements for the role and work location.

Nice to have

• Bachelor’s degree in Recreational Therapy.
• Certified Therapeutic Recreation Specialist (CTRS) or Activity Professional Certification (APC).
• Spanish, Mandarin, or Cantonese bilingual ability.
• Knowledge of PACE, CMS, and state regulatory requirements.
