Location: Chicago, Illinois, United States. The role requires approximately 75%+ field travel across DuPage, Will, Lake, and Cook Counties, reliable transportation, and flexibility for occasional evenings and weekends.
Annual on-target earnings: $80,000β$88,000, including 20% tied to uncapped monthly commission.

What you will do
- Conduct consultations that identify client needs, urgency, and readiness, then guide families toward timely care decisions.
- Coordinate service starts, align expectations, arrange care setup, and mitigate onboarding risks.
- Maintain ongoing relationships through reassessments, care plan adjustments, and quality visits.
- Partner with scheduling, operations, and Care Professionals to support consistent service delivery.
- Manage client portfolios using data and systems, track performance, and maintain accurate documentation.
- Support referral relationship growth and the local market reputation as needed.
Requirements
- At least 1 year of experience in a consultative, client-facing role such as healthcare, senior care, social work, or care coordination.
- Ability to guide clients through decisions and deliver measurable outcomes such as conversion, retention, or service adoption.
- Strong relationship management skills and experience adapting support to evolving client and family needs.
- Experience leading structured discovery, needs-assessment, recommendation, and next-step conversations.
- Ability to collaborate cross-functionally, resolve execution challenges, and maintain operationally accurate records.
- Ability to spend approximately 75% of time in the field, with reliable transportation and flexibility for occasional evenings and weekends.
Nice to have
- Experience working with older adults and families.
- Background in Healthcare Administration, Social Work, Gerontology, or a related field.
