Salary: USD 34.50–55.20 per hour

What you will do

• Assess patients and families, identify ongoing care needs, and initiate timely referrals.
• Collect and analyze information for new patients and coordinate multidisciplinary care, appointments, and clinic scheduling.
• Collaborate with patients, families, physicians, ancillary staff, and clinical teams to develop coordinated care plans.
• Educate patients, families, and caregivers about diagnoses, therapies, testing, and care processes.
• Support transitions of care, delegate appropriately, and promote patient safety.
• Serve as a clinical resource and maintain effective communication across clinical and administrative disciplines.

Requirements

• Completion of an accredited Registered Nurse program.
• Current RN license in the state of Illinois required.
• Current nursing experience within the past five years.
• Current BLS Certification.
• Strong interpersonal, communication, critical-thinking, and care-coordination skills.

Nice to have

• Bachelor of Science in Nursing.
• Oncology Certified Nurse certification.
