CCM Specialist (RN)

What you will do

• Develop and monitor care coordination and CCM protocols in alignment with CMS guidelines.
• Ensure accurate documentation and billing compliance for all CCM and TCM services.
• Educate clinical staff on comprehensive assessments, individualized care plans, and post-discharge follow-up.
• Design and implement innovative care models to enhance coordination across inpatient and community settings.
• Track and report key performance indicators to identify opportunities for process improvement.
• Oversee the onboarding and training of new care coordinators and clinical team members.

Requirements

• Must be a Registered Nurse (RN).
• Must be based in the USA.
• Demonstrated knowledge of CMS guidelines and value-based care strategies.
• Experience with ACO operations and clinical quality improvement.
• Ability to analyze patient outcomes and readmission data to refine workflows.
