Behavioral Health Care Coach (Behavioral Health)

Location: Remote South Carolina, with 75–90% travel to Berkeley, Dorchester, Charleston, Colleton, Hampton, Jasper, and Beaufort counties. Candidates should reside within or just outside these counties. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $53,700–$72,600 per year

What you will do

• Assess and evaluate members’ behavioral health, wellness, and care needs.
• Establish individualized care plans and implement interventions aligned with members’ personal goals.
• Identify and resolve barriers to effective care and support members in the least restrictive setting.
• Coordinate services with providers, payers, interdisciplinary teams, families, and community resources.
• Monitor and evaluate case management plans and document member care activities.
• Conduct frequent face-to-face visits with Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.

Requirements

• Bachelor’s degree in a health and human services field.
• At least 2 years of related experience.
• Must be a South Carolina resident and live within or just outside the listed service counties.
• Strong verbal, written communication, and interpersonal skills.
• Ability to use electronic information applications and software programs.
• Must have a separate room with a locked door for a private home office and protection of member PHI/HIPAA information.
• Valid state driver’s license and personal vehicle liability insurance are required.

Nice to have

• Experience working with Medicare and Medicaid recipients.
• Experience with electronic case-note documentation and multiple computer systems.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.

• Full-time, 40-hour work week, Monday through Friday, 8:00 AM to 5:00 PM.
• Field work comprises 75–90% of the role, with mileage reimbursement for eligible work-related travel.
