Salary: $120,000–$130,000 per year

What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments for patients at home.
• Perform health risk assessments, medication reviews, cognitive screenings, and preventive care interventions.
• Collaborate with in-home clinical staff to evaluate vital signs, arrange blood draws, and address patient needs.
• Coordinate medications, specialist referrals, community resources, and ongoing patient care.
• Provide patient education and document clinical findings accurately.

Requirements

• Board certification as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in New York.
• Prior experience performing wellness visits.
• Strong written and verbal communication skills, professionalism, compassion, and accountability.
• Ability to work independently and collaboratively, including solving problems with limited notice.
• Comfort using laptops, mobile devices, charting tools, and HIPAA-secure messaging applications; successful completion of a national background check and clinical license search.
