What you will do

• Prepare, compound, inspect, and dispense sterile IV medications and related supplies in accordance with USP 797/800.
• Design and monitor patient-specific care plans, assess suitability for home infusion, and track relevant laboratory values and therapeutic drug levels.
• Recommend medication or regimen adjustments to physicians, nurses, patients, and caregivers using pharmacokinetic principles.
• Monitor pharmacy operations, ancillary personnel, inventory, controlled substances, recalls, shortages, and medication safety events.
• Provide pharmacotherapy expertise, patient counseling, staff education, and continuity-of-care support.
• Maintain regulatory, accreditation, documentation, quality-control, performance-improvement, and on-call responsibilities.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in the state of practice; Pharm.D. or RPh.
• Working knowledge of sterile IV admixtures, aseptic technique, and USP 797/800 guidelines.
• 1–2 years of clinical pharmacy experience in home care, hospital, or another infusion setting.
• Ability to communicate effectively in English, verbally and in writing.
• Ability to lift and push up to 50 pounds and perform extended standing, bending, lifting, and physical activity.
